For good and valuable consideration, the receipt of which I acknowledge, I grant Percolate Studios and its successors, assigns, and licensees the irrevocable, perpetual, worldwide right to record, use, and reproduce my name, likeness, image, voice, statements, and performance (the "Materials") in the above production and in any related promotional, marketing, and publicity materials, in whole or in part, in all media now known or later devised — including theatrical exhibition, streaming and on-demand platforms, broadcast, and the internet and social media. Percolate Studios owns all rights in the Materials, including copyright, and may edit, alter, and combine them with other content at its sole discretion, without further approval, compensation, or credit to me.

I release and hold harmless Percolate Studios and its successors, assigns, licensees, employees, and agents from any claims arising out of the recording or use of the Materials, including claims for defamation, invasion of privacy, right of publicity, or false light. I confirm that I am at least 18 years old (or that a parent or guardian has signed below on my behalf), that I have read and understood this release, and that I sign it freely.
